To all parents:

A new Parental Consent form & Medical Release form MUST BE FILLED OUT EVRY YEAR on each child in Children's Church, Flight Crew 2, and flight Crew Youth. This form must be on file in order that the child may participate in any church sponsored event.

Please call for a form or pick one at church.
